What a response includes

The caption block: application number, examiner, art unit, confirmation number, and the right rule for a final or non-final action.
A claim listing with a 37 CFR 1.121(c) status identifier on every claim.
A Remarks heading under every rejection and objection, with the examiner's stated basis and the references applied.
For each §103, the examiner's stated reason to combine. If there isn't one, we say so.
